Comparison of the treatment efficacy in patients with venous leg ulceration treated with conservative methods, autogenic and allogenic skin grafts.

Introduction:
It is commonly known that venous leg ulceration is a major health problem that affects a lot of patients and has a great impact on their everyday life. In treatment firstly conservative methods are applied and when the ulcers fail to heal, autogenic or allogenic (Apligraf) skin grafting may be considered. The aim of our work was to compare the effectiveness of treatment in patients with venous leg ulceration treated with conservative methods,
autogenic and allogenic skin grafts.

Material and methods:
Twenty-nine randomly selected patients with venous leg ulcers ambulatory treated and hospitalized in Public Clinic Hospital No 4 in Lublin in 2008 were examined and interviewed with self-constructed questionnaire. Out of 29 examined patients there were 25 women (86,2%) and 4 men (13,8%) at average age of 67,9 (67,6 for women and 68,3 for men).

Results:
Patients were divided into three groups according to the way of treatment: conservative methods-17 patients (58,6%), autogenic skin grafting-7 patients (24,1%) and allogenic skin grafting – Apligraf - 5 patients (17,2%). Pain intensity was definitely lower in patients treated with skin grafts (83,3%). A complete wound healing was observed in all patients (100%) treated with Apligraf, in 4 patients (51,1%) treated with autogenic graft and in 5
patients (29,4%) treated with conservative methods. Most of the responders considered skin grafts to be more effective method of treatment in comparison to conservative methods, adequately 75,7% patients treated with autogenic skin grafts and 100% patients treated with Apligraf.

Conclusions:
Conservative ways of treatment seem to be less effective taking into consideration the ulcers healing and pain ailments reduction. Apligraf appears to indicate a greater efficacy than the autogenic skin grafting and conservative methods.
